Senate To Take Up Sotomayor Confirmation
The full Senate is set to begin debating the Sonia Sotomayor nomination today, with a final vote as early as Thursday. Sotomayor is on track to win confirmation by a comfortable margin, with all or nearly all Democrats plus a few Republicans supporting her.
Obama's Birthday Ahead
Today is President Obama's 48th birthday -- but in terms of his schedule, it's a regular work day at the White House. Obama will meet at 11:05 a.m. ET with Dave Rehbein, the National Commander of the American Legion. At 12 p.m. ET, he and Vice President Biden will have lunch with the Senate Democratic Caucus. Obama will meet with Biden at 3:45 p.m. ET, and with Sec. of Defense Robert Gates at 4:30 p.m. ET.
Biden's Day Ahead
Vice President Biden will have a breakfast meeting at the Naval Observatory with U.S. Special Representative for Afghanistan and Pakistan Ambassador Richard Holbrooke. At 11 a.m. ET, he will have a meeting with members of the White House economic team to discuss the economic impact of the Recovery Act. In the afternoon, he will join President Obama's 12 p.m. ET lunch with the Senate Democrats, meet with Obama at 3:45 p.m. ET, and join the 4:30 p.m. ET meeting with Sec. of Defense Robert Gates.
Bill Clinton Arrives In North Korea
Former President Bill Clinton has arrived in North Korea, to negotiate the release of two American journalists who were arrested and sentenced to 12 years imprisonment in March, for illegally entering the country. Analyst Scott Snyder, of the Asia Foundation, said that Clinton will likely succeed -- or else suffer a great political loss: "For someone at his level to go without a prior assurance of some kind would be to risk a huge loss of face."
House Lefties Angered At Dismissive Comment By Pelosi
The Hill reports that House progressives are offended that Speaker Nancy Pelosi dismissed their threat to oppose the health care bill as being too weak. "It didn't take us very seriously," said Rep. Lynn Woolsey (D-CA). "She may be overlooking the strength behind the 60, and there are more who are absolutely committed to a robust public option."
Obama Looks To Heal Dem Infighting
Roll Call reports that President Obama will be working to heal rifts within the Democratic Party on health care, by reminding Senators today that their political fortunes are tied to his own. "We all hang together or we all hang separately," said a senior Senate Democratic aide.
Cash For Clunkers Extension Uncertain In Senate
The New York Times reports that the White House's proposed $2 billion extension of the Cash For Clunkers program faces an uncertain future in the Senate, where Republicans could potentially hold up passage. A spokesman for Senate Minority Leader Mitch McConnell (R-KY) said that some Republicans had "real objections to it," though McConnell himself has not directly spoken out against the extension.
Administration To Tout Economic Policy Successes
The Washington Post reports that President Obama and members of the Cabinet are set to travel to various swing states this week, to declare that recent economic numbers show that the administration's policies are working. But there are still some pitfalls -- while the rate of decline has drastically slowed down, the economy is still not yet in positive territory, and jobs are still being lost.

Some Republicans have a real objection to the fact that the program is working. Some of those people who still have jobs who've been hoarding cash like mad are letting go of some of it. GM might meet its sales targets for the quarter. Ford is actually running out of cars and will have to make more if the program continues. Oh yeah, and there will be a minor reduction in the national carbon footprint extending over what would otherwise have been the remaining useful life of a bunch of old cars that could have been on the road for another ten or twenty years.

Cash for Clunkers is a great opportunity to demonstrate to Senate Republicans that using a vote against cloture to kill a bill only works when the "leadership" chooses to allow it. The program is popular, and it is a very public success in stimulating auto sales. Moreover, the Senate is hot to trot home for a holiday.
If the Republicans want to talk the bill to death, and look like fools to the public, and keep themselves in Washington in August, let them.

The 'innovative' idea of a 'pay for value / outcome' pack came after the CBO had previously pointed out this health care reform wouldn't work without 'fundamental' change in the out of date system. It is said that as much as 30 percent of all health-care spending in the U.S. -some $700 billion a year- may be wasted on tests and treatments that do not improve the health of the recipients, and this 700 billion dollars a year can cover a lot of uninsured people.
The expected Benefits of this 'innovative idea' are as follows ;
1. Meet the objective of revenue-neutral.
Supporters of the agreement say it could save the Medicare System more than $100 billion a year and 'improve'
care, that means more than $1trillian over next decade, and virtually needs no other resources including tax on the
wealthiest. Supposedly even the 'conservative' number of such savings might be able to meet the objective of
revenue-neutral.
2. Quality and affordability.
If you are a physician, and your pay is dependant upon your patient's outcome, you will most likely strive to
prescribe the best medicine earlier in the process, let alone skipping the wasteful, unnecessary treatments.
3. No intervention in decision-making.
The innovative idea of 'a pay for outcome' will more likely prompt team approach and decision, as at Myo clinic.
Under the 'pay for outcome' pack, for good reason, best practices as 'recommendations' would simply help them
make a better decision, and the government won't still have to meddle in the final, actual decision-making
process as a non-expert.
4. Speed up the introduction of IT SYSTEM.
The pay for 'Outcome' pack is most likely to expedite the introduction of Health Care IT SYSTEM.
The synergy effect of the combined Health Care IT & a pay for 'outcome' system may allow the clinicians to
'correctly' diagnose and effectively treat a patient earlier in the process so that it can measurably scale back the
crushing lawsuits and deter the excuse for unnecessary cares to make fortunes.
5. Accelerate the progress in medical science, in return, it saves more money.
6. Settle the regional disparity.
7. Reduce the emergency room visits & save immense costs.
Public health insurance plans such as Medicare and Medicaid paid for more than 40 percent of U.S. emergency
room visits in 2006, according to government figures released recently. Many experts say reducing these hospital
visits would be an important way to lower the enormous, and growing, expense of U.S. health care.
